Sales Tax Exemption Form:

In compliance with the State of Michigan sales tax laws, we must collect Michigan sales tax on orders purchased by Michigan based clients or on orders purchased by other than Michigan clients that are shipped to a Michigan address unless we have a completed signed tax exemption certificate form in our files.

Sales tax forms must be updated yearly. In order to keep a non-taxable status, please complete and mail your certificate the first of every year. We cannot be responsible for reminding customers to keep their tax status updated, and cannot refund sales tax charged on an order.
